Extracorporeal Shockwave Therapy (ECSWT) / (Low Intensity Shockwave Therapy (LiSWT)

Extracorporeal Shockwaves have been used successfully in Europe for over a decade for a variety of conditions, including heart disease, kidney stones and orthopedic conditions.

The technology works by accelerating a projectile in the hand piece which generates a mechanical pressure wave. This wave is transferred to the human body by an applicator head and which radiates out in the tissue. The mechanical energy is absorbed by the tissue and decreases as the distance from the application site increases. The mechanical stress causes cavitation resulting in reactions in the tissue.

The benefits of the therapy’s mechanical stimulation include: increased local blood flow, cellular activity such as the release of substance P, prostaglandin E2, NO, TGF β, VEGF, and almost certainly other inflammatory cytokines, the breakdown of calcific deposits and adhesions, and finally relaxation of connective tissue.

Low-level laser (light) therapy (LLLT) is a fast-growing technology used to treat a multitude of conditions that require stimulation of healing, relief of pain and inflammation, and restoration of function. The photons are absorbed by mitochondrial chromophores in skin cells. Consequently, electron transport, adenosine triphosphate (ATP) nitric oxide release, blood flow, reactive oxygen species increase and diverse signaling pathways get activated. Stem cells can be activated allowing increased

tissue repair and healing. In dermatology, LLLT has beneficial effects on wrinkles, acne scars, hypertrophic scars, and healing of burns. LLLT can reduce UV damage both as a treatment and as a prophylaxis. In pigmentary disorders such as vitiligo, LLLT can increase pigmentation by stimulating melanocyte proliferation and reduce depigmentation by inhibiting autoimmunity. Inflammatory diseases such as psoriasis and acne can also benefit.

Ablative laser

This is a wounding laser, which removes thin layers of skin. Types of ablative treatments include the carbon dioxide (CO2) laser and the erbium laser.

Nonablative laser

This is a non-wounding laser, which stimulates collagen growth and tightens underlying skin. This includes many types of lasers as well as intense pulsed light (IPL) devices.

Another type of laser resurfacing, fractional photothermolysis (Fraxel), is available in ablative and non-ablative forms. Although non-ablative laser resurfacing is less invasive and requires less recovery time, it’s less effective than is ablative laser resurfacing.

Mesenchymal Stem Cell Exosomes

Exosomes are extracellular vesicles ranging between 30-150 nm that are produced by virtually every cell type as a means of intercellular communication They contain proteins (growth factors), mRNA (blueprint for protein production) and micro RNA (on a switch for specific protein production), all contained within a membrane similar to their parent cells that protects exosomal proteins and RNA from degradation until they are delivered to the target cell.

MSC exosomes are very different than exosomes found in adult bone marrow, which has a preponderance of hematopoietic stem cell exosomes, or amniotic fluid, which has primarily maternal epithelial cell exosomes. The distinct ability of MSC exosomes to induce connective tissue synthesis may be the basis of many of the remarkable clinical benefits that have been observed with stem cell therapy.

As we age, the number and function of the MSCs in our tissues declines sharply. Aged autologous progenitor cells produce about 30% of the cytokines and significantly different miRNAs compared with perinatal MSCs. This substantial difference in secretome confers a significant advantage of perinatal MSC exosomes over exosomes from aged autologous or allogeneic progenitor cells.

Structural Wharton’s Jelly

Wharton’s jelly is a gelatinous substance in the umbilical cord that provides cushioning and support to the umbilical vein and arteries. The cushioning and protective elements from Wharton’s jelly consist of a network of structural proteins, pericytes, mesenchymal stem cells, cytokines, chemokines and growth factors. Hyaluronic acid is the most abundant component of the glycosamnioglycans found in the umbilical cord matrix. Hyaluronic acid is also a key factor in the viscoelastic properties in synovial fluid of joints.

Cord Blood

“Cord blood” refers to blood from the umbilical cord and placenta. Usually, the cord is traditionally discarded after birth. Compared to bone marrow, cord blood cells are “immature”, which means they haven’t been exposed to a disease. Stem cells in the umbilical cord are different from bone marrow stems cells, which have been the treatment standard for stem cell therapies. The cells in cord blood include hematopoietic progenitor cells (HPC) and hematopoietic stem cells (HSC), which are also found in bone marrow and peripheral blood.

Why Human Amniotic Tissues?

Clinicians have used human amniotic tissues for over a century as a biologic dressing in a broad range of therapeutic applications. Human amniotic tissues are an abundant source of collagen, as well as other proteins and cytokines. Human amniotic tissues are “immuneprivileged” and therefore rarely cause an immune response in the human body. In addition, this tissue has been shown to have anti-inflammatory, anti-microbial and anti-adhesive properties. The collagens in the amniotic tissue provide a structural tissue matrix for cellular attachment. While providing structural support to cells, the extracellular matrix assists in the migration and proliferation of the patient’s own cells to the site of injury or defect.

The cells inherent in the human placental tissues have been found to be broadly multipotent, capable of differentiating into adipogenic, osteogenic, myogenic, endothelial, neurogenic and hepatic cell lineages.
